Insurance
I accept many health insurance plans for payment at contracted rates. I also have a few sliding scale hours per week for individuals facing economic hardship. Other payment options can be discussed with patients on an individual basis.
I am an in-network provider on the following insurance panels:
– Aetna
– BlueCross and BlueShield
– Moda Health
– PacificSource
– Premera
– Providence
– Regence
Call if you have questions about insurance companies I have not listed here. I can also bill insurance as an out-of-network provider.1 I will provide you with a standard insurance reimbursement form (‘superbill’) with which you to can request reimbursement from your insurance provider.
1 Please keep in mind that many plans have deductibles and co-pays. It is best to check these costs with your insurance provider when possible. Payment of your copay or deductible is due at the time of service or can be negotiated at time of intake.
Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.
I’d recommend asking these questions to your insurance provider to help determine your benefits:
• Does my health insurance plan include mental health benefits?
• Do I have a deductible? If so, what is it and have I met it yet?
• Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
• Do I need written approval (authorization) from my primary care physician in order for services to be covered?
Payment
I accept Cash, Check, Visa, Mastercard, or Discover. HSA and other payment options are also possible.
Cancellation Policy
If you are unable to attend a session, please make sure to cancel at least 24 hours beforehand. Otherwise, you may be charged for the full rate of the session or fraction thereof.
